How to apply for a Library Card
Patrons must come into the library and fill out and sign a Borrower Card Application form to register for a library card.
Library cards are issued free of charge to all Wisconsin residents with a current library card in their home town. Patrons must provide a picture ID, either a driver’s license or state ID, on behalf of themselves and/or minor children. Birthdate is required on application forms.
Applicants under 18 years of age must have a parent or guardian sign the application form.
The person whose signature is on the card application is responsible for all materials borrowed from the library under that library card number according to the policies of the lending library.
Notes about your new Library Card
Library cards expire after one year. Cards may be renewed after verification of application information.
Patrons with lost cards should notify the library as soon as possible and request a replacement card.
